10 Early Warning Signs of Kidney Failure You Should Never Ignore
Kidney disease is often called a silent killer because by the time symptoms appear, kidney function has often dropped below 25%. Recognising early warning signs can mean the difference between slowing kidney disease and ending up on dialysis. Here are 10 signs to watch for — and what to do if you spot them.
1. Persistent Fatigue and Weakness
Healthy kidneys produce erythropoietin (EPO) — a hormone that signals bone marrow to make red blood cells. Failing kidneys produce less EPO, leading to anaemia and tiredness that doesn’t go away with rest.
2. Swelling in Feet, Ankles, or Around Eyes
When kidneys can’t excrete sodium and water properly, fluid builds up. Persistent puffiness around the eyes (especially in mornings) or swollen feet/ankles is a classic warning sign.
3. Foamy or Bubbly Urine
Excess protein leaking into urine creates persistent foam — a key sign of glomerular damage. If foam doesn’t clear after flushing, it warrants investigation.
4. Changes in Urination Pattern
Urinating much more (especially at night), much less, or noticing blood (pink/cola-coloured urine) all signal kidney dysfunction.
5. Itchy Skin That Won’t Resolve
Toxin buildup (uraemia) causes severe itching. If you’ve ruled out skin conditions but itching persists, check kidney function.
6. Loss of Appetite and Metallic Taste
Uraemic toxins blunt taste and reduce appetite. Many CKD patients describe a metallic taste, especially with meat.
7. Nausea and Vomiting
Toxin buildup also causes nausea, particularly in the morning. Persistent nausea without GI cause should trigger kidney evaluation.
8. Difficulty Concentrating / Brain Fog
Anaemia plus uraemic toxins reduce mental clarity. Memory issues, difficulty focusing, or feeling ‘foggy’ can be early CKD signs.
9. Muscle Cramps Especially at Night
Electrolyte imbalances (low calcium, high phosphate) and dehydration cause cramping, especially in calves at night.
10. High Blood Pressure That’s Hard to Control
Kidneys regulate BP. Failing kidneys cause persistent, treatment-resistant hypertension. If your BP is rising despite multiple medications, check kidney function.

- Urine routine + ACR (albumin-creatinine ratio) — detects protein/albumin leak

- Blood pressure log — at home, twice daily for a week
- HbA1c — if diabetic, controls a major CKD driver
