AV Fistula vs Catheter for Dialysis — Which Vascular Access?

AV fistula vs tunnelled catheter for dialysis — infection risk, longevity, mortality. Why fistula-first matters. Renacare interventional nephrology: +91 98181 83957.

AV Fistula vs Catheter for Dialysis — Which Vascular Access is Right for You?

Before starting hemodialysis, your nephrologist needs a way to connect your bloodstream to the dialysis machine. This is called vascular access. The two main options — AV fistula and tunnelled catheter — have very different long-term implications for your safety and quality of life.

What is an AV Fistula?

An arteriovenous (AV) fistula is created surgically by connecting an artery to a vein, usually in the wrist or upper arm. Over 6-8 weeks, the vein ‘matures’ — thickens and dilates — making it suitable for repeated needle insertion during dialysis.

What is a Tunnelled Catheter?

A tunnelled dialysis catheter is a soft tube placed in a large central vein (typically internal jugular). It’s tunnelled under the skin for stability. It can be used for dialysis immediately after insertion.

AV Fistula vs Catheter — Side by Side

  • Infection risk: Fistula very low; Catheter HIGH (catheters are #1 cause of dialysis bloodstream infections)
  • Longevity: Fistula 5-15+ years; Catheter typically <2 years
  • Time to use: Fistula 6-8 weeks maturation; Catheter immediate
  • Quality of life: Fistula better (showering, swimming OK); Catheter restrictions on water exposure
  • Mortality association: Fistula linked to lower mortality on dialysis; Catheter higher mortality

Why Most Nephrologists Prefer AV Fistula

The mantra in nephrology is “fistula first.” Lower infection risk, longer lifespan, better long-term mortality. Catheters are reserved for emergency dialysis, fistula maturation period, or when fistula creation isn’t anatomically feasible.

When You Need a Catheter

  • You need urgent dialysis and have no fistula yet
  • Your fistula is still maturing (bridge access)
  • You have failed multiple fistula attempts
  • Your veins are too small/damaged for fistula creation

Renacare’s Vascular Access Approach

At Renacare, our fistula-first policy means we plan vascular access early — ideally before you need dialysis. Dr. Anil Prasad Bhatt and team perform AV fistula creation and tunnelled catheter placement as part of our interventional nephrology services.
